死亡证明
A death certificate is a confidential document, 因此, only the immediate family – the spouse, the biological or adoptive parents, the biological or adoptive grandparents, the legal guardian, 的兄弟姐妹, or the adult children of the deceased can request and obtain a death certificate that shows the cause of death.
If you have a tangible interest in a matter pertaining to the decedent, you can request a certified copy of the death certificate; however, the cause of death will be redacted.
To obtain a death certificate you must present a government issued picture I.D. (driver's license, military I.D.，国家颁发.D.，或护照).

Requests by mail must include the following information: the full name of the deceased, 死亡日期, 死亡之县, and county of residence. Please include a check or money order and a self-addressed, stamped envelope. Also, a clear, legible photo copy of government picture I.D. 是必需的.
